Baylor Licensed Practical Nurse (LPN) โ€“ ICF/IIDDepartment: Intermediate Care Facilities (ICF/IID)
Reports To: ICF/IID Program Coordinator / ICF Program Director
FLSA Status: Non-Exempt
Schedule Type: Baylor (Weekend Extended Shifts) โ€“ 6:00 am-6:00 pm and 6:00 pm-6:00 am options. Position SummaryThe Baylor Licensed Practical Nurse (LPN) serves as the clinical team lead on designated weekend shifts within the Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID). This position provides direct nursing care, medication administration, and oversight of resident aides to ensure safe, person-centered, and regulatory-compliant care in accordance with ICF/IID standards.Essential Duties and Responsibilities
  • Serves as team lead during assigned Baylor shift and supervises at least two ICF/IID Resident Aides.
  • Works cooperatively with Program Director, RN, Program Coordinator, Enrichment Coordinator, and interdisciplinary staff.
  • Implements active care plans (CFAs, IPPs, BSPs) and prepares required documentation and reports.
  • Administers medications (oral, subcutaneous, intramuscular) and documents on MARs.
  • Monitors residents for adverse reactions and reports findings to RN.
  • Performs LPN-skilled procedures including catheter care, feeding tube support, and oxygen administration.
  • Assists Enrichment Coordinator in therapeutic, social, and recreational programming.
  • Collaborates with RN, Program Coordinator, and Dietician to implement nutritious meal planning and special diets.
  • Takes residents to medical appointments as necessary.
  • Maintains cleanliness and safety of the home in compliance with licensing and organizational policies.
  • Maintains medical and household supplies, including stocking, ordering, and disposing of expired items.
  • Documents resident visits, outgoing calls, and other required communications.
QualificationsCurrent, unrestricted LPN license in good standing.
Graduate of accredited practical nursing program.
Experience in ICF/IID, developmental disabilities, or long-term care preferred.
Supervisory experience preferred.Physical RequirementsAbility to lift, transfer, and reposition residents as needed.
Ability to stand and walk for extended periods.
Ability to assist with mobility and medical equipment.
