Registered Nurse – Home Health


URGENT NEED!

We are seeking an experienced Registered Nurse (RN) to provide high-quality home health care to patients throughout West Virginia. This role is ideal for a compassionate, organized nurse with strong home health experience and a commitment to patient-centered care.


Position Details

  • Schedule: Monday–Friday, 8:00 AM–5:00 PM
  • On-Call: At least 1 weekend per month
  • License: Active WV RN License
  • Productivity: 30 points per week
  • Mileage: $0.54 per mile
  • Service Areas: Monongalia, Preston, Tucker, Grant, and Wetzel Counties


Required Qualifications

  • Home Health experience
  • HCHB experience
  • OASIS experience
  • Active WV RN license
  • Strong clinical assessment and documentation skills


Responsibilities

  • Assess patients' health and develop, implement, and evaluate individualized care plans
  • Provide ongoing disease management, health maintenance, and case management
  • Monitor changes in patient condition and communicate updates to physicians and other healthcare providers
  • Coordinate care with interdisciplinary teams, inpatient facilities, and discharge coordinators
  • Manage medications, physician orders, and related documentation
  • Complete required nursing documentation and maintain accurate patient records
  • Schedule and follow up on health-related appointments and implement appropriate recommendations
  • Supervise and support LPN/LVN staff as needed
  • Train new employees and provide ongoing health and safety education
