Baptist Health Home Health/Hospice RN Case Manager provides, manages and coordinates care and services for an assigned caseload of patients.
Other information:
Education required: Graduation from an approved program of Nursing.
Required: Current Arkansas RN License.
Required: Current Arkansas Driver's License.
Preferred: Minimum 1 year of nursing experience. Home Health/Hospice experience preferred.
This job will be authorized 80.00 hours bi-weekly.
