The Care Team Hospice is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our growing team! This is an excellent opportunity for a nurse who is passionate about providing hands-on patient care while supporting individuals and families through every stage of the hospice journey.
As a Hospice LPN, you'll work alongside an experienced interdisciplinary team to deliver skilled nursing care, monitor patient needs, and help ensure patients receive the comfort, dignity, and support they deserve. Key Responsibilities
โ€ข Provide skilled nursing care under the direction of the Registered Nurse while following each patient's individualized plan of care.
โ€ข Monitor patient conditions, recognize changes in health status, and communicate clinical updates to the care team.
โ€ข Assist with symptom management, medication administration, treatments, and patient comfort measures.
โ€ข Complete accurate and timely documentation of patient care and observations within the electronic medical record (EMR).
โ€ข Collaborate with physicians, Registered Nurses, caregivers, and the interdisciplinary team to deliver compassionate, high-quality hospice care.
Qualifications
โ€ข Active Licensed Practical Nurse (LPN) license
โ€ข Previous nursing experience required; hospice, home health, long-term care, or skilled nursing experience preferred.
โ€ข Strong clinical assessment, communication, and patient care skills.
โ€ข Ability to work independently while effectively collaborating with an interdisciplinary team.
โ€ข Experience with electronic medical records (EMR) preferred.
Field Role Requirements
โ€ข Valid driver's license, reliable transportation, and current automobile insurance.
โ€ข Ability to pass a criminal background check and Motor Vehicle Record (MVR) screening.
โ€ข Current CPR certification.
โ€ข Current TB testing in accordance with agency policy.
