Job description

Position Title: RN Home Health - Local or Travel

Location: Denver, CO

Schedule: Monday - Friday | 8:00 AM - 5:00 PM (40 hours/week)


On-call info: On-call required if requested- on-call needs are subject to change based on staff and branch needs
Productivity Units: 30 units
Qualifications: HCHB, Oasis, and HH experience required

Productivity: 30 units per week

Employment Type: 13 Weeks Contract (Extension or Full time available)

Required Qualifications

  • Current Registered Nurse (RN) license in the state of Colorado / Compact License

  • 2+ years of Home Health nursing experience

  • Experience with OASIS documentation

  • Experience with Homecare Homebase (HCHB) software

  • Current CPR certification

  • Valid driver's license

  • Reliable transportation with active insurance

  • Minimum 1-2 years of clinical nursing experience

Preferred Qualifications

  • CHPN certification

  • Bachelor of Science in Nursing (BSN)

Education Requirements

  • Associate Degree in Nursing (ADN) or Nursing Diploma - Required

  • Bachelor of Science in Nursing (BSN) - Preferred

Responsibilities

  • Provide skilled nursing care to patients in their homes

  • Complete comprehensive patient assessments and develop care plans

  • Accurately complete and submit OASIS documentation

  • Document patient care using Homecare Homebase (HCHB)

  • Educate patients and families on treatment plans and disease management

  • Participate in required on-call rotation schedule
