An Autism Case Manager is a professional who coordinates services and support for individuals with autism spectrum disorder (ASD) and their families. They assess clients' needs, develop care plans, connect families to resources, and monitor progress to ensure the best possible outcomes. Autism Case Managers often work closely with healthcare providers, schools, and community organizations to advocate for clients and help navigate complex systems. Their goal is to improve the quality of life for individuals with autism by providing guidance and ensuring access to necessary services.

Job description

Marklund is built on our mission of making everyday life possible for individuals with profound disabilities. Our services are designed to enhance the lives of our residents in a meaningful way, but we can't do it alone.

We are seeking a dedicated Nurse Case Manager for our Community Day Service program to join our diverse team in Geneva and be a part of our life-changing mission.

The salary of this position is $82,000 - $87,000 based on qualifications and experience.

Joining Marklund is about being part of something bigger. It's about belonging to a community that believes in the wholeness of each person, and works together to create happy, safe and purpose-filled lives for those individuals that we serve.

Marklund is a place where you can thrive professionally, and grow in your career. Whether you seek work variety, scheduling consistency or career advancement, you will find your home here. Your journey includes innovative benefits, flexible opportunities, competitive pay rates for nurses with experience, and extensive training to help new graduates launch your nursing career.

According to one of our nurses, Cass, Marklund stands about from other facilities:

“The reason I became a nurse was to help, nurture, and show love to other fellow beings who were in need, and I am able to live that dream at Marklund.

What sets Marklund apart for me is the homelike setting as well as the client-staff ratio.  I feel as though I'm providing care for someone in their home which inspires me to keep care more personal. It's a very warm and inviting atmosphere, which makes me feel as if I'm going to my home away from home.  Also because of the working ratios the quality of care provided is much higher, which is something you can feel proud of.

If you want to enrich the lives of others by something as small as a smile or gentle touch, Marklund is the place for you.  If you like going home feeling that you received as much or more than you gave, Marklund is the place for you. If you like knowing you may be the one constant in someone's life, Marklund is the place for you.”

Who We Are:

Marklund is a nonprofit organization that provides residential, educational and therapy services to infants, children, teens and adults with serious and profound developmental disabilities and special healthcare needs.  With two residential locations - the Marklund Hyde Center in Geneva, and the Marklund Wasmond Center in Elgin, it is currently home to nearly 178 residents.  The Marklund Day School offers Multi-Needs and Life Skills programs for children and young adults with multiple medical needs, and those diagnosed on the autism spectrum.  In addition, our Community Day Services program at each site is a therapeutic day program for adults who reside at Marklund and those who come in from the community.

Through clinical excellence, caring practice and extraordinary teamwork, Marklund nurses boldly advance the practice of nursing every day. As a Nurse Case Manager you will make a difference in the lives of our residents in the following ways:

  • Serve as the Administrative and Clinical Coordinator of all nursing activities for the day program.
  • Administer medications, tube feedings and treatments as prescribed.
  • Supervise nursing and direct care staff.
  • Work with staff, parents and regulatory bodies to ensure all appropriate measures are taken in relation to resident care.
  • Plan, develop and coordinate Quality Improvement Programs to enhance the quality of resident care.
  • Monitor Infection Control Program, in accordance with policies and procedures, to ensure the highest degree of quality resident care and safety can be maintained at all times.

Please note that this job summary is not designed to cover or contain a comprehensive listing of activities, duties, or responsibilities that are required of the employee for this job. Duties, responsibilities and activities may change at any time with or without notice.

As an equal opportunity employer, Marklund respects and supports diversity, equity and inclusion as part of our work environment.
