Home Health Registered Nurse (RN) – Bilingual (Spanish/English)

Bilingual Requirement:
This position requires the ability to effectively communicate with Spanish-speaking patients and families. Fluency in both English and Spanish is required to support patient care needs.

As a Home Health Registered Nurse, here’s a big-picture view of what you’ll do:

  • Complete initial and ongoing comprehensive assessments.
  • Coordinate for patient/client discharge from services.
  • Provide care according to the patient’s/client’s plan of care/service plan.
  • Participate with the patient/client in clarifying mutually agreed upon goals and incorporate these goals into the plan of care/service plan.
  • Conduct home visits as assigned, assessing the patient’s condition, progress toward goals, providing patient care, teaching and other skilled interventions as ordered.
  • Develop a care plan in collaboration with Case Manager, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions, including the patient and family in the planning process.

A few must-haves for Registered Nurses:

  • Graduate of an accredited nursing program and active RN license in Arizona
  • One (1) home health experience preferred but not required
  • Valid Arizona Fingerprint Card
  • CPR Certification (demonstration course required)
  • Must have a valid TB screening
  • Proof of flu vaccination (October–March, optional)
  • Knowledge of state and federal healthcare laws and regulations
  • Demonstrated clinical proficiency, critical thinking skills, good communicator and compassionate
  • Fluency in English and Spanish required due to the needs of our Spanish-speaking patient population to ensure safe, effective care and communication
