Transitional Care Coordinator / Home Health Liaison

Hartford County, Connecticut
Nurses At Home

Nurses At Home is looking for a personable, driven Transitional Care Coordinator / Home Health Liaison to help grow our presence throughout Hartford County while making the transition from hospital, rehab, or physician care back home as smooth as possible for patients and their families.

This is a relationship-focused role for someone who enjoys being out in the community, meeting people, developing referral relationships, and helping patients get connected with the care they need at home.

What You'll Do
  • Build and maintain relationships with hospitals, skilled nursing facilities, assisted living communities, physician offices, rehabilitation facilities, senior communities, and other referral partners throughout Hartford County.

  • Serve as a primary point of contact for discharge planners, case managers, social workers, physicians, and other referral sources.

  • Educate referral partners about Nurses At Home's skilled home health services and how we can assist their patients after discharge.

  • Meet with patients and families when appropriate to explain home health services and help coordinate a safe transition home.

  • Identify and develop new referral relationships throughout the territory.

  • Maintain regular communication with existing referral partners and make sure they receive timely follow-up on referrals.

  • Work closely with our intake, clinical, scheduling, and leadership teams to help ensure referrals are handled quickly and patients are admitted without unnecessary delays.

  • Help identify barriers that may prevent a patient from successfully transitioning home and communicate those issues to the appropriate clinical or operational team.

  • Attend community events, networking opportunities, facility meetings, and other healthcare-related events.

  • Track referral activity, relationship development, and opportunities within the Hartford County territory.

  • Be a visible and professional representative of Nurses At Home in the communities we serve.

Who We're Looking For

We're looking for someone who is outgoing, organized, dependable, and comfortable building relationships with healthcare professionals.

Healthcare experience is strongly preferred. Experience in home health, hospice, skilled nursing, assisted living, hospital case management, physician outreach, healthcare marketing, or post-acute care would be especially valuable.

The right person should:

  • Be comfortable walking into a facility or physician office and developing relationships.

  • Communicate well with patients, families, clinicians, and healthcare professionals.

  • Understand the importance of responsiveness and follow-through in healthcare referrals.

  • Be able to work independently and manage a territory.

  • Have strong organizational and time-management skills.

  • Be comfortable using technology and documenting referral and outreach activity.

  • Have a valid driver's license, reliable transportation, and the ability to travel throughout Hartford County.

Clinical licensure is not required, although candidates with a clinical healthcare background are encouraged to apply.
