Job description

Legacy Healthcare Services is searching for an experienced and motivated Physical Therapist (PT) to join our newly created Better at Home team. We’re committed to providing comprehensive healthcare services in the comfort of our patients' homes to positively impact quality of life by redefining the delivery of care.  Come join a team that is empowering senior adults to lead a life filled with purpose, safety, and joy!

  • Pay Rate: Based on a Per Visit Rate
  • Coverage Location: Wake County, NC

Responsibilities: 

  • Provide physical therapy services to patients according to physician's plan of care and as defined in the state Physical Therapy Practice Act. 
  • Assess and evaluate therapeutic, rehabilitative/functional status to develop, implement and direct physical therapy programs that focus on rehabilitation. 
  • Evaluate the patient’s home environment, assess patients for muscle strength, mobility, gait, ROM potential for rehab.  
  • Direct patients and families/caregivers in the use and care of therapeutic appliances. 
  • Initiate physical therapy program and instruct other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient. 
  • Identify patient and family/caregiver needs for other home health (PCA) services and refer as necessary.  
  • Prepare and submit progress summaries based on the attainment of goals.  
  • Supervises PTAs according to organization policy and state regulations.
