Responsibilities
  • Perform skilled nursing visits, assessments, admissions, recertifications, resumptions of care, and discharges in patients' homes.

  • Develop, implement, and update individualized Plans of Care.

  • Complete OASIS assessments and clinical documentation accurately and on time.

  • Monitor patient progress and communicate changes with physicians and the interdisciplinary team.

  • Educate patients and caregivers on disease management, medications, treatments, and safety.

  • Coordinate care with physicians, therapists, home health aides, and other healthcare professionals.

  • Supervise Home Health Aides in accordance with state and federal regulations.

  • Promote safe, compassionate, high-quality patient care while helping patients remain safely at home.

Qualifications
  • Graduate of an accredited Registered Nursing (RN) program.

  • Current, unrestricted Ohio Registered Nurse license.

  • Current CPR certification.

  • Minimum of one year of nursing experience required.

  • Home Health experience is preferred but not required.

  • OASIS experience is a plus.

  • Valid driver's license, reliable transportation, and auto insurance.
