At Choice Health at Home, you'll enjoy the flexibility of working one-on-one with patients in their homes while being supported by an experienced clinical team dedicated to your success.

What You'll Do

As a Home Health Physical Therapist, you will evaluate, develop, and implement individualized treatment plans that improve mobility, reduce pain, restore function, and help patients safely remain in their homes.

Responsibilities include:

  • Perform comprehensive physical therapy evaluations and establish individualized plans of care.
  • Develop and implement treatment programs to improve strength, balance, mobility, and functional independence.
  • Reassess patient progress and modify treatment plans to achieve the best outcomes.
  • Educate patients and caregivers on therapeutic exercises, safety techniques, and injury prevention.
  • Collaborate with physicians, nurses, and interdisciplinary team members to coordinate patient care.
  • Maintain accurate, timely documentation in accordance with agency policies and regulatory standards.
  • Follow infection prevention and patient safety protocols during every visit.
  • Promote exceptional patient care while representing Choice Health at Home with professionalism and compassion.
Qualifications
  • Graduate of an accredited Physical Therapy program
  • Current Physical Therapist (PT) license in the state of practice
  • Current CPR Certification
  • Valid driver's license and reliable transportation with current auto insurance
  • Minimum one year of Home Health experience preferred
  • HomeCare HomeBase experience preferred
Physical Requirements
  • Ability to stand, walk, bend, kneel, squat, climb, and lift throughout the workday.
  • Ability to lift, carry, push, or pull up to 100 pounds with appropriate assistance when necessary.
