Experience: A minimum of 3 years' experience with assisted living/memory care residents and programming is preferred.

License/Certification: Licensed as a Registered Nurse, multi-state licensure may be required.

Overview:

The Registered Nurse-Assessment Nurse will assist the Director of Nursing with the development and ongoing administration of the patient's assessments. The Registered Nurse may at times administer nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Nursing.

Duties and Responsibilities:

1. Provides services in accordance with the plan of care.

2. Makes the initial evaluation visit and regularly reevaluates the patient's nursing needs.

3. Initiates the plan of care and necessary revisions.

4. Prepares clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner as per Agency policy.

5. Coordinates services.

6. Informs personnel of changes in the condition and needs of the patient.

7. Counsels the patient and family/significant others in meeting nursing and related needs.

8. Participates in and presents in-service programs.

9. Processes orders and notifies physician of patient needs and changes in condition. Completes certification/recertification orders and discharge summaries.

10. Determines the amount and type of nursing needed by each individual patient.

11. Conducts patient care conferences on patients assigned to his/her care.

12. Participates in utilization review of medical records as assigned.

13. Completes and submits assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with company defined time frames.

14. Participates in other professional development activities as required.

Knowledge and Skill:

1. Must have excellent verbal skills and communication abilities.

2. Ability to work effectively within a team based environment.

3. Strong time management and organizational skills.

4. Ability to calculate figures related to budgetary items, staffing needs, medication dosages, and amounts such as percentages and area.

5. Knowledge of applicable health care and employment Federal and State regulations.

6. Must have excellent computer skills, including; Word, Excel, Outlook, Power Point, and Property Management Systems.

Qualifications:

1. Must be able to clearly and proficiently communicate with people of all ages, while maintaining an energetic and positive attitude.

2. Must be able to read and write English, and follow verbal and written instructions.

3. Minimum of two (2) years in similar position, with retirement living or home-care background preferable.

4. Ability to exercise initiative and independent judgment

5. Ability to work flexible hours including weekends and evenings.

6. Ability to read, analyze, and interpret human service periodicals, professional journals, technical procedures, and governmental regulations.

7. Ability to write reports, business correspondence, procedure manuals.

8. Ability to effectively present information and respond to questions from managers, clients, customers, and the general public.

9. Graduate of an approved school of professional nursing and currently licensed as RN in the state(s) in which practicing
