Position Summary:
Under the supervision of the Program Director the Care Coordinator is responsible for anything related to the coordination of care for all ROWI clients. The Care Coordinator will coordinate individualized service including orientation, transportation, doctor visits, and schedule.
Duties and Responsibilities:
  • Ensures programs and services are implemented at the highest standards and clients receive the highest level of care.
  • Responsible for client supervision and ensuring they attend designated groups throughout programming.
  • Facilitate assigned groups and activities as designated by the Facility Manager.
  • Conduct client orientation for all admits on day of admission or assign to another staff if not available. Including UA screening, PHQ9 and other assessments as directed.
  • Ensure compliance with Joint Commission Standards.
  • Document shift reports and share pertinent information with the team.
  • Coordinate the transportation schedule for all clients and drive clients as needed.
  • Document all transportation and group activity in EMR.
  • Administer and submit documentation in EMR for client Urine Analysis.
  • Assist with facility cleanliness as needed.
  • Responsible for coordinating client lunch daily.
  • Serve as a role model to treatment staff, clients, employees, and guests through a positive, caring, and professional attitude.
  • Maintain the confidential nature of clients and related activities.
  • Comply with the dress code.
  • Complete tasks in a timely and accurate manner.
  • Attend designated staff and in-service meetings.
  • Any additional duties as assigned by the management team.
  • Ability to maintain clear boundaries with clients and staff, set limits and manage clients when in need of direct supervision. Maintains appropriate and professional interactions with clients and staff.
  • All documentation in EMR must be submitted within 24 hours.

Basic Education and Experience Required:

  • Minimum of 1-year working with kids in a therapeutic setting
  • Experience with junior high or high school students is preferred.
