SummitWest care is a local non-profit home health agency serving Mesa County. We're seeking a compassionate, motivated Physical Therapist Assistant (PTA) to provide in-home therapy services under the direction of our licensed Physical Therapists.

This position offers flexibility, independence, and the opportunity to build meaningful relationships with patients while helping them improve mobility, safety, and quality of life in their own homes. Whether you're seeking full-time or part-time hours, you'll be supported by a collaborative team that values both excellent patient outcomes and work-life balance for our clinicians.

Responsibilities

  • Provide skilled therapy services in patient homes
  • Carry out established treatment plans involving therapeutic exercise, gait training, balance activities, and functional mobility intervention
  • Monitor and communicate patient progress and changes to supervising PT
  • Encourage patient participation and reinforce goals
  • Complete timely, accurate documentation in EMR
  • Collaborate with therapists, nurses, caregivers, and other care team members

Qualifications

  • Current Colorado Physical Therapist Assistant (PTA) license in good standing
  • Graduate of an accredited Physical Therapist Assistant program
  • Valid driver's license, reliable transportation, and auto insurance
  • Strong communication and documentation skills
  • Ability to work independently and adapt professionally in home-based setting
