What You Will Do
As the Triage and Coordination Float Nurse, you will provide rotational pediatric and adult coverage, delivering telephone triage, developing and coordinating care plans, offering patient education, managing scheduling and referral needs, and supporting follow-up care for patients served by IHI.
The Opportunity
  • Triage incoming telephone calls and manage care needs resulting from after-hours calls.
  • Facilitate timely communication of nursing assessments and recommendations aligned with organizational clinical communication guidelines and triage policies to providers, team members, patients, and families to ensure comprehensive patient support.
  • Under the direction of a physician, instruct patients on emergent care, medications, and report patient response to treatment back to the physician.
  • Communicate laboratory, test, and imaging results to patients and families.
  • Coordinate outgoing referrals and complete scheduling for requested tests and procedures.
  • Advocate for patient and family needs while monitoring for healthcare-related outcomes.
  • Participate in the organization's clinical structure and multidisciplinary approach to patient care.
  • Manage prescriptions and refills under physician direction, including factor-replacement therapies.
  • Navigate the electronic health record (EHR) effectively, including accurate chart review and documentation.

Performance Requirements
Knowledge:
  • Medical practice telephone-triage protocols and familiarity with nursing assessment and diagnosis, including medical terminology, anatomy, and physiology.
  • Healthcare systems, structures, and functions, as well as available community resources.
  • Basic laboratory procedures and the use of healthcare technology, equipment, and supplies.
  • Documentation of standards for charting and medical records, along with federal and state regulations governing the release of health information.
  • Development of home care plans for patients who do not require immediate in-person or emergency treatment.

Skills
  • Uses sound judgment, critical thinking, and effective prioritization in clinical decision-making.
  • Demonstrates strong time-management, problem-solving, multitasking, and care-coordination skills.
  • Utilizes electronic health records to review patient history and guide appropriate clinical decisions.
  • Handles highly sensitive interpersonal situations with professionalism and empathy.
  • Makes accurate triage decisions and responds quickly and calmly in urgent or emergency situations.

Abilities
  • Engages patients and families in developing and implementing individualized care plans.
  • Works independently while maintaining accountability and clinical accuracy.
  • Communicates clearly, calmly, and compassionately with patients and families.
  • Elicits essential information needed to make immediate and appropriate clinical evaluations.
  • Documents in the medical record with correct spelling, punctuation, and clear presentation of the situation and plan of care.

Requirements
  • Minimum 2 years of general nurse experience; preferrable in collaborative chronic disease patient care or hematology.
  • Valid RN license, able to practice in the state of Indiana.
  • Current BLS certification.
  • All IHI employees are expected to enable multi-factor authentication via their personal smartphone or smart device to access IHI systems as a requirement of their role.
