JOB SUMMARY
Performs responsible clinical care in collaboration with the unit physician and under approved protocols, manages the medical aspects of care in the observation, assessment, diagnosis, intervention, evaluation, rehabilitation, care and counsel, and health teachings of persons who are ill, injured or infirm or experiencing changes in normal health processes; and in the promotion and maintenance of health of prevention of illness in a clinical setting.
PHYSICIAN ASSISTANT
EDUCATION & EXPERIENCE
Minimum Qualifications:
  • Completion of a physician assistant program accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA)
  • No experience required

LICENSES, REGISTRATIONS OR CERTIFICATIONS
Required:
  • Certification by the National Commission on Certification of Physician Assistants (NCCPA)
  • Licensed Physician Assistant by the Texas Physician Assistant Board/Texas Medical Board (TPAB/TMB)

Current students who have not yet completed their physician assistant program or recent graduates who have not yet obtained their NCCPA certification or TPAB/TMB physician assistant license are eligible to apply for this position. However, official employment is contingent upon the successful completion of an accredited physician assistant program, NCCPA certification and TPAB/TMB physician assistant licensure.
NURSE PRACTITIONER

Minimum Qualifications:
  • Completion of a graduate level (or higher) nurse practitioner program accredited by the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE)
  • No experience required

LICENSES, REGISTRATIONS OR CERTIFICATIONS
Required:
  • Certification by the American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP)
  • Licensed Registered Nurse (RN) by the Texas Board of Nursing (TBON)
  • Licensed Advanced Practice Registered Nurse (APRN) in a nurse practitioner clinical area of specialization by the Texas Board of Nursing (TBON)

Current RNs who have not yet completed their nurse practitioner program or recent graduates who have not yet obtained their ANCC/AANP certification or APRN nurse practitioner license are eligible to apply for this position. However, official employment is contingent upon the successful completion of an accredited nurse practitioner program, ANCC/AANP certification and APRN nurse practitioner licensure.
Minimum Qualifications:
  • Master's or Doctoral degree in Nursing or Physician Assistant Studies from an accredited program.
  • Current Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA-C) license in the State of Texas.
  • National certification in Adult-Gerontology Primary Care, Family Practice, or related specialty.

About the Division of Geriatrics
The Division of Geriatrics at UTMB is dedicated to improving the health, independence, and quality of life of older adults through excellence in clinical care, education, and research. With 8 faculty, 8 advanced practice providers and 2 to 4 fellows per year, the Division provides comprehensive geriatric care across diverse settings, including ambulatory clinics, inpatient consult services, long-term care facilities, and the home environment through our Home-Based Primary Care Program.
The Home-Based Primary Care Program is a cornerstone of the Division's mission-delivering compassionate, high-quality care to homebound older adults. This innovative program enables faculty and trainees to provide continuity of care, promote aging in place, and reduce unnecessary hospitalizations by addressing the complex medical, functional, and social needs of each patient.
The Division also leads UTMB's Acute Care for Elders (ACE) Unit, a dedicated inpatient program designed to optimize outcomes for hospitalized older adults. The ACE Unit integrates a multidisciplinary approach-combining the expertise of physicians, nurses, therapists, pharmacists, and social workers-to promote functional recovery, prevent complications, and ensure smooth transitions of care.
In addition to its clinical excellence, the Division is committed to training the next generation of geriatricians through its ACGME-accredited Geriatric Medicine Fellowship Program, which offers robust inpatient and outpatient experiences, exposure to interdisciplinary care models, and mentorship from nationally recognized faculty. The Division also plays a vital role in medical education at all levels, integrating geriatric principles into the curriculum for medical students and residents across specialties.
Research and scholarly activity are integral to the Division's work, with ongoing projects focused on healthy aging, dementia care, falls prevention, polypharmacy, and healthcare delivery innovations. The Division partners closely with the Sealy Center on Aging, creating a vibrant academic environment that fosters collaboration and discovery.
UTMB is seeking a full-time Advanced Practice Provider within the Division of Geriatrics-Home-Based Primary Care Program. This position emphasizes patient-centered primary care services to medically complex, homebound, and functionally limited older adults across home and nursing facility settings. The program's goal is to deliver high-quality, coordinated healthcare that promotes independence, enhances quality of life, and reduces avoidable hospitalizations.
Job Duties:
  • Provide comprehensive primary care services to geriatric patients in home and nursing facility settings, focusing on prevention, chronic disease management, and functional well-being.
  • Performs individualized history and physical examinations for diverse populations inclusive of social determinants of health.
  • Orders and interprets diagnostic testing in accordance with relevant health concerns of the patient.
  • Obtain laboratory testing-includes blood specimen draws, catheterization, flu swabs, and joint injections.
  • Precepting and mentoring school of nursing and school of medicine students.
  • Develops a comprehensive plan of care that encompasses evidence-based practice guidelines, practice protocols, Medicare, Medicaid, and other state and federal regulations.
  • Prescribes appropriate pharmacological and non-pharmacological treatments.
  • Provides appropriate education for current health diagnoses as well as anticipatory guidance for health promotion and disease prevention applicable to the individual's health and socioeconomic status.
  • Monitors plan of care for cost-effectiveness and the achievement of expected improvements, while openly seeking feedback from patients, significant others, colleagues, allied health, appropriate referrals, and collaborating physician to enhance quality outcomes.
  • Works independently to full scope of practice in compliance with the Texas Nurse Practice Act with acknowledgement of collaborating physician availability for consultation and collaboration.
  • Collaborate with interdisciplinary teams-including nursing, pharmacy, social work, and rehabilitation-for optimal care of older adults.
  • Contribute to curriculum development, clinical skills training, and geriatric-focused academic programs.
    Preferred Qualifications:
  • Experience providing primary care to geriatric or medically complex populations in home or long-term care settings.
  • Experience in an academic medical center or teaching environment.
  • Strong interpersonal and communication skills with a collaborative, team-oriented approach.
