Net Team Lead Intensive Services
You want to make a difference in the lives of our clients; we need your expertise to carry out our mission.
As the Network Team Lead-Intensive Services you will be responsible for the planning, management and supervision of daily operations and delivery of services within the ACT/FACT teams. In this position you will provide direct services to patients and will also assist in managing the assignment of referrals, coordinating services, monitoring staff availability, ensuring timeliness delivery of services including assessments and person-centered recovery plans, approving clinical documentation, and gathering and providing data required by the ACT/FACT models. The position will also work closely with community partners who serve mutual clients.
Minimum Requirements
Minimum Education: Bachelor's Degree
Defined Education: Health and Human services field
Years' Experience: Four (4) years
Defined Experience: Health and Human Services field
License/Certifications: Valid Texas Driver's License
