Description

Epic Nursing Services is seeking a compassionate and motivated Community Case Worker to support our Adult Family Care (AFC) and Group Adult Foster Care (GAFC) programs. In this role, you'll help adults age 22 and older remain safe and independent in their homes through case management, care coordination, home visits, and community outreach.

Responsibilities:

  • Conduct initial and ongoing member assessments
  • Complete home visits and develop care plans
  • Coordinate services with caregivers, healthcare providers, and community resources
  • Support and educate caregivers
  • Maintain accurate documentation
  • Build referral relationships through community outreach

Qualifications:

  • High school diploma required
  • At least 1 year of case management or care coordination experience
  • 2 years of experience working with elderly or disabled individuals preferred
  • Knowledge of community resources and medical terminology
  • Strong communication and organizational skills
  • Valid driver's license and reliable transportation required
