Pediatric Transitional Nurse - Registered Nurse (RN) and Licensed Practical Nurse (LPN)

Memphis, TN | Biweekly Pay | Day & Night Shifts Available | On-Call Responsibilities as Needed

Pay Rate:

  • RN $37-$40/hr
  • LVN $32-$36/hr

This role is ideal for nurses who are confident working withhigh-acuity patients andskilled in trach, vent, G-tube, and emergency management.

As a Transition Nurse, you'll play a critical role ensuring safe, successful discharges, preventing readmissions, providing interim patient coverage, and supporting continuity of care until permanent staffing is established.

What You'll Do

  • Provide skilled pediatric nursing 1:1 care in patients' homes
  • Perform patient assessments and ongoing evaluations -Administer medications, treatments, and therapies
  • Support hospital-to-home transitions for high-acuity patients
  • Educate and support parents, families, and caregivers
  • Assist with discharge support and care coordination
  • Train and support permanent field nurses during transitions
  • Provide first-shift and interim coverage as needed
  • Collaborate with Clinical Care Managers and interdisciplinary teams
  • Respond appropriately in emergency situations and changes in patient condition

Qualifications

Required:

  • Active, unrestricted Registered Nurse or Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) license (TN)
  • Current CPR certification
  • Ability to lift and assist pediatric patients as needed
  • Experience with Trach care, Ventilator management, and TPN
  • Emergency response management

Preferred:

  • Pediatric home health experience
  • High-acuity pediatric patients (preferred)

Schedule & Travel

  • Full-time position, with guaranteed hours
  • Day and Night shifts available
  • On call and Staffing coverage responsibilities as needed
  • Field-based role with local travel
