Job description

Pay Range:

Under general supervision, manages communication in region(s) to provide guidance, from the regional perspective to ANTHC, and support patients in their respective region(s). Provide patient navigation services to assist patients regarding information about reservations, travel, appointment referrals, and tracking follow-up and outcomes. Be a liaison between community residents and ANMC/ANTHC with the goal to enhance local knowledge of ANTHC services, direct requests for technical assistance and be a resource for general questions/concerns with ANTHC services.

Accountabilities

  • Collaborate with the Tribal Partner in the region to develop processes and solutions to improve and enhance referral visits to ANMC.

  • Identify as the regional liaison to the Consortium, be the local resource for questions and concerns with direct contact to ANTHC/ANMC to quickly resolve issues that may arise, and facilitate communication from ANTHC to the appropriate Tribal Partner's personnel when requested.

  • Communicate with ANMC providers, case managers and regional healthcare organizations to ensure patient appointments, travel arrangements, referrals, lodging, transportation, and other logistical matters are completed. Facilitate interaction and communication with health care staff and providers. Track patient follow-up and care coordination.

  • Communicates with other departments at ANTHC to coordinate as needed. Utilizes a tracking system to maintain an appropriate database of issues requiring follow-up and assures appropriate follow- up of customer-owners.

  • Confirm insurance eligibility, communicate and coordinate with ANTHC Health Benefits Specialists to assist patients with applying for benefits and keeping insurance information current.

  • Provide outreach education to patients. Make sure patient education materials are distributed in the clinics.

  • Identifies and develop relationships with the Tribal Partner's personnel; offer educational sessions to inform their personnel of the field office role and services and to encourage utilization of the Field office services. Proactively looks for ways to support the Tribal Partner when not busy with other ANTHC responsibilities.

  • Proactively connect with ANTHC staff traveling to the region to inform them of issues being resolved or requests.

  • Promote educational opportunities to expose both village school students and other residents to potential careers and opportunities they can pursue and resources available through ANTHC

  • When requested, collaborates with the Tribal Partner to resolve issues with Video Teleconferences (VTC) appointments between ANMC and the patients.

  • Maintain appropriate records and prepares reports as required.

  • Performs other duties as assigned.

Education, Certifications, and Licenses Required

  • Associates Degree in business or related field. Progressively responsible customer service, travel experience, or related work may be substituted on a year-by-year basis for the college education.

  • Must possess valid Driver's License and clean driving record

  • Must have reliable transportation.

Experience Required

  • Non-supervisory- One (1) year relevant professional work experience in community health or case management.

Knowledge, Skills & Abilities:

  • Demonstrated knowledge of the Alaska Tribal Health System, including service delivery, travel pattern and referral systems.

  • Knowledge of Alaska Natives, Alaska Native cultures and rural Alaska.

  • Knowledge of state, federal, and Tribal health care programs.

  • Knowledge of state, federal and public/private insurance, including Medicaid/Medicare.

  • Knowledge of customer service concepts and practice.

  • Knowledge of basic medical terminology and clinic systems.

  • Knowledge of the Privacy Act of 1974 and HIPAA Privacy Rule Act of 1966.

  • Skill in patient education and outreach.

  • Skill in working with a clinical care team.

  • Skill in using video teleconference equipment, or ability to learn technical skills.

  • Skill in working with and communicating effectively with a variety of professional and skill levels; such as medical providers, case managers, health educators.

  • Skill in working independently or as a team member.

  • Demonstrated skill in assessing and prioritizing multiple tasks, projects and demands.

  • Demonstrated skill in operating a personal computer utilizing a variety of software applications and platforms
