Family First Healthcare Services is seeking a skilled, compassionate, and motivated Speech Therapist (SLP) to join our growing home health team. Position Summary

We are seeking a licensed Speech-Language Pathologist (SLP) to provide high-quality speech therapy services to patients in a home health setting. You will assess communication, cognitive, and swallowing needs; develop individualized treatment plans; and deliver evidence-based interventions to help patients improve function, safety, and quality of life.

The ideal candidate is patient-centered, organized, clinically strong, and committed to delivering exceptional home-based care.

Key Responsibilities
  • Conduct comprehensive speech, language, cognitive, and swallowing evaluations in the home

  • Interpret patient histories, assessments, and physician orders to develop individualized plans of care

  • Provide therapeutic interventions for speech, language, voice, cognition, and dysphagia as appropriate

  • Educate patients and caregivers on treatment strategies, safety techniques, and carryover exercises

  • Modify treatment plans based on patient progress and evolving clinical needs

  • Collaborate closely with nurses, physicians, case managers, and interdisciplinary team members

  • Maintain accurate, timely, and compliant documentation in the EMR

  • Ensure adherence to federal, state, and agency regulatory standards

  • Stay current with evidence-based practices and professional standards in speech-language pathology

Qualifications
  • Master’s degree in Speech-Language Pathology from an accredited program

  • Active Florida Speech-Language Pathologist license in good standing

  • ASHA Certificate of Clinical Competence (CCC-SLP) preferred

  • Strong communication, interpersonal, and patient engagement skills

  • Ability to work independently and manage a home health caseload

  • Compassionate, detail-oriented, and committed to high-quality patient care

  • Proficient with computers and electronic medical record documentation

  • Experience with Alora Plus EMR preferred

  • (Preferred) Prior home health experience
