We are seeking an Admissions Coordinator who thrives at the intersection of clinical coordination, regulatory compliance, and family support.
This is not a passive administrative desk job. You will be the engine of our intake pipeline, managing hospital referrals, navigating insurance verifications, and serving as the warm, reassuring guide for families making critical healthcare transitions.
The Role: Driving the Admissions Pipeline
Working closely with our Director of Marketing & Community Relations and our clinical team, you will manage the inquiry-to-admission process from start to finish, ensuring high census and absolute regulatory accuracy.
Manage Referrals with Speed: Efficiently review and process incoming clinical referrals from hospital discharge planners and electronic referral platforms (e.g., CarePort, Allscripts).
Navigate Payer Sources: Conduct initial insurance verifications, obtain managed care pre-authorizations, and confidently guide families through Medicare, Medicaid, and private pay parameters.
Coordinate Clinical Care Reviews: Partner closely with our Director of Nursing and clinical team to ensure rapid medical necessity reviews and clear alignment on care requirements.
The First Impression: Act as the primary point of contact for families, giving professional tours of our clinical areas and coordinating all required admission paperwork with empathy and precision.
Maintain Compliance: Ensure all admissions documentation complies with federal, state, and internal regulatory standards, updating the facility CRM in real time.
What You Bring to the Table (Required Qualifications)
Please Note: Prior experience in a healthcare or post-acute care environment is required for this role.
SNF/LTC Admissions Experience: Minimum of 1–2 years of direct admissions or intake coordination experience in a Skilled Nursing Facility (SNF), Sub-Acute Rehabilitation, or Long-Term Care setting.
Payer Source Literacy: Strong working knowledge of Medicare, Medicaid, and commercial managed care insurance guidelines, pre-certification processes, and terminology.
Tech-Savvy & Fast: Experience utilizing electronic referral networks (CarePort/Curaspan, NaviHealth) and healthcare CRM software.
The Post-Acute Hustle: Ability to multi-task in a high-volume, fast-paced environment where response time to hospital discharge planners directly impacts census goals.
Crisis Compassion: Exceptional communication and interpersonal skills, with the emotional intelligence to de-escalate stressful situations for families navigating a medical crisis.
