Reporting to the Program and Resource Manager, you will be responsible for overseeing the overall success and health of Easter Seals Alberta's programs for Alberta South region. This position requires a strong background in delivering program excellence. The ideal candidate will have strong interpersonal and communication skills, along with extensive knowledge of community resources and needs based assessment tools.

Responsibilities:
As part of our team, you will play a critical role in empowering individuals and families through empathetic support, collaborative partnerships, and innovative service delivery.
Client Support & Service Navigation
Provide clear, accurate, and accessible information on disability supports and services, including Persons with Developmental Disabilities (PDD), Family Managed Services (FMS), and Family Supports for Children with Disabilities (FSCD) agreements.
Support individuals, families, and professionals in navigating systems, accessing resources, and coordinating services to meet their unique needs.
Guide clients through needs-based assessments and co-create personalized service plans that promote independence, inclusion, and wellbeing.
Connect clients to community and provincial resources, including financial supports, adaptive equipment, mental health services, and other disability-related programs.
Assist clients in accessing and using digital tools (e.g., OCR system, Alberta.ca accounts, digital signatures) to support efficient service access.
Capacity Building & Education
Empower clients and families to confidently manage their supports by providing coaching on employer responsibilities, including hiring, training, and managing staff.
Offer guidance on financial reporting and compliance requirements for FMS and FSCD agreements.
Deliver engaging learning opportunities for clients, families, and professionals on topics such as FSCD and PDD programs, employer responsibilities, Financial Literacy, and best practices.
Program Delivery & Continuous Improvement
Support the development and implementation of key planning tools such as Individual Support Plans (ISPs), Transition to Adulthood Plans (TAP), Behavioural Support Plans (BSPs), and related processes.
Monitor program performance, track outcomes, and gather client feedback to identify trends, challenges, and opportunities for improvement.
Translate insights and data into actionable recommendations that enhance service quality and client outcomes.
Develop and implement processes that improve workflow efficiency and strengthen service delivery.
Ensure programs align with Easter Seals Alberta's strategic plan and commitment to continuous improvement.
Partnerships & Community Engagement
Build and maintain strong relationships with community organizations, service providers, and stakeholders.
Collaborate with partners to enhance service coordination, expand resources, and address shared challenges.
Represent Easter Seals Alberta at community events, presentations, and interagency meetings to raise awareness and strengthen community impact.
Leadership & Team Support
Provide guidance, mentorship, and oversight to Resource Coordinators within your region.
Foster a collaborative, supportive team environment that reflects our values and commitment to excellence.
Equity, Inclusion & Advocacy
Integrate inclusive practices into all aspects of service delivery.
Actively address barriers to ensure equitable access for all individuals and families.
Support clients in building natural supports and meaningful connections within their communities.
Required Qualifications:
Post-secondary education in Social Work (or an equivalent combination of education and relevant experience).
Minimum of 2-3 years' experience in case management, service navigation, or supporting individuals and families in a human services environment.
Demonstrated knowledge of Alberta disability support systems, including PDD, FSCD, and FMS, or the ability to quickly build this expertise, is an asset.
Experience working with individuals with disabilities and/or chronic medical conditions, with a strong understanding of person- and family-centered approaches.
Strong communication skills (written and verbal), with the ability to explain complex information clearly, compassionately, and accessibly. Individuals with skills in more than one language would be an asset.
Proven ability to build relationships and work effectively with diverse individuals, families, professionals, and community partners.
Experience facilitating training, workshops, or educational sessions for clients, families, or staff.
Strong organizational and time management skills, with the ability to manage multiple priorities in a fast-paced environment.
Demonstrated problem-solving skills, sound judgment, and the ability to take initiative in addressing client and program needs.
Ability to analyze information, identify trends, and contribute to continuous improvement of programs and services.
Proficiency with Microsoft Office and digital tools (experience with SharePoint or similar platforms) is an asset.
Experience developing and implementing Individual Support Plans (ISPs), Transition to Adulthood Plans (TAP), and Behavioural Support Plans (BSP's) is an asset.

Position details:
This position is 40 hours/week in office, reports to the Program and Resource Manager.
Must have Leadership skills and be past the RSW provisional hours requirement
Valid Driver's license and access to a reliable vehicle is a requirement for this position as travel is required within the South region when needed.
A clear Enhanced Police Information Check and Vulnerable Sector Search are conditions of employment.
