Schedule & Coverage
  • Shift: Friday at 5:00 PM to Monday at 8:00 AM.
  • Availability: Must remain available for phone triage and field deployment throughout the designated shift.
Position Overview
The Weekend Admissions and On-Call Nurse (RN) provides comprehensive, high-quality home health services from Friday evening through Monday morning. This dual-responsibility role manages the after-hours triage helpline and conducts in-person clinical visits, including Start of Care (SOC) admissions, Resumption of Care (ROC) assessments, and emergency PRN visits escalated via triage. This position utilizes an integrated AI ambient-documentation platform to capture real-time clinical notes and streamline OASIS drafting within the EMR.

This is reducing paperwork time on start of care assessments by over an hour per patient and improving accuracy while allowing more face-to-face time with each patient.

Key Responsibilities
1. Clinical Assessments, Admissions & AI Documentation
  • SOC & ROC Management: Complete comprehensive, timely assessments for Start of Care (SOC) and Resumption of Care (ROC) admissions according to Medicare conditions of participation and agency policies.
  • AI Platform Utilization: Leverage the agency's AI clinical scribe/documentation platform during field assessments to capture patient interactions, generate draft clinical narratives, and accelerate OASIS completion.
  • Quality & Compliance: Review, edit, and validate all AI-generated draft documentation to ensure strict clinical accuracy, regulatory compliance, and alignment with the patient's true clinical presentation before final submission in the EMR.
  • Care Planning: Develop, implement, and update evidence-based plans of care in collaboration with physicians and the interdisciplinary team.
2. Triage & On-Call Coverage
  • Telephone Triage: Manage the after-hours helpline, assessing patient, caregiver, and referral inquiries using professional clinical judgment.
  • Escalation & PRN Visits: Deploy to the field to perform urgent, hands-on PRN visits when a patient's condition requires clinical intervention beyond telephonic guidance.
  • Care Coordination: Collaborate with on-call physicians, pharmacies, and families to manage acute changes in patient status and prevent unnecessary hospitalizations.

Qualifications
  • Experience: Minimum of 1-2 years of direct home health experience is required. Strong foundational knowledge of OASIS items and logic.
  • Licensure: Active, unrestricted Registered Nurse (RN) license in the state of practice.
  • Certifications: Current Basic Life Support (BLS) / CPR certification.
  • Technical & Core Skills:
    • Comfortable adopting advanced clinical technology, specifically AI-assisted documentation and ambient scribing tools.
    • Demonstrated ability to work autonomously with minimal supervision.
    • Strong critical thinking and triage skills to differentiate urgent vs. non-urgent clinical scenarios.
    • Proficiency with Home Health EMR software and mobile applications.
  • Logistics: Valid driver's license, reliable transportation, and automotive insurance for required field visits.
  • Preferred: Prior home health experience
