About Qualicare

Position Summary
The Registered Nurse will assess client needs, develop and update individualized Plans of Care, provide skilled nursing services, and help ensure that care meets professional, regulatory, and agency standards.
The RN may also supervise and support Certified Home Health Aides and collaborate with physicians, therapists, caregivers, and other healthcare professionals to provide coordinated care.
Responsibilities
  • Conduct initial and ongoing in-home nursing assessments
  • Develop, review, and update individualized Plans of Care
  • Provide direct nursing care based on client needs
  • Monitor clients and identify changes in condition
  • Communicate significant findings to families, physicians, and the care team
  • Complete accurate and timely clinical documentation
  • Coordinate services with healthcare professionals and caregivers
  • Conduct supervisory visits and evaluate care provided by home health aides
  • Provide education and clinical guidance to caregivers and families
  • Follow infection-control, safety, HIPAA, and agency requirements
  • Report incidents, concerns, and changes in client condition
Qualifications
  • Active New Jersey Registered Nurse license - In good standing
  • Current CPR certification
  • Strong clinical assessment and critical-thinking skills
  • Home care, home health, geriatric, rehabilitation, or community nursing experience preferred
  • Experience supervising Certified Home Health Aides preferred
  • Knowledge of medication management, infection control, documentation, and care planning
  • Current negative TB test or chest X-ray
  • Ability to complete all required background checks and screenings
  • Excellent communication, organization, and customer-service skills
  • Compassionate, dependable, professional, and detail-oriented
  • Ability to work independently
  • Comfortable using technology for scheduling and clinical documentation
  • Ability to travel throughout Bergen County
  • Valid driver’s license and reliable transportation preferred
