Make a Difference every day!!!!ย  We are looking for an On-Call Weekend Nurse for Hospice and Home Healthย 

GENERAL PURPOSE:ย 

The weekend On-Call Staff Nurseย is responsible for nursing call Friday 8:00 am through Monday 8:00 am. The Weekend Staff Nurse is first call during this time.ย  Scheduled visits may be assigned during daytime hours.ย ย 

QUALIFICATIONS:ย 

  • Registered nurse with current licensure to practice professional nursing in the state.ย 

  • Graduate of an accredited school of nursing.ย 

  • Maintains a current CPR certification.ย 

  • Minimum of two (2) years' experience, at least one (1) of which is in the area of public health or home care nursing.ย 

  • Demonstrates an ability to work with other health care clinicians, development of home plan of care, knowledge of third-party reimbursement, and ability to negotiate payment rates.ย 

  • Self-directed and able to work with minimal supervision.ย 

  • Demonstrate excellent observation verbal and written communication skills.ย 

  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.ย 

  • Must be capable of performing the job functions of this position with or without accommodations.ย 

ESSENTIAL JOB FUNCTIONS:ย 

  • Ensures quality and safe delivery of services, within the guidelines of accepted nursing practice and state and local law.ย ย 

  • Assumes Case Management responsibilities for individual patients/family including:ย 

  • Comprehensive Physical Assessmentย 

  • Pain Assessment and management.ย 

  • Symptom control management as it relates to terminal illness.ย 

  • Weekend On call visiting Staff Nurse.ย 

  • Completes required documentation (i.e. assessments, care plans, clinical notes, agency tracking forms, etc.) in a timely manner in accordance with agency policies.ย ย 

  • Provides weekend coverage and may assist during times of excessive nursing absenteeism.ย ย 

  • Is directly responsible to the Patient Care Coordinator and works under the direction of the patientโ€™s attending physician.ย ย 

  • Informs the ADON of unusual or potentially problematic patient/family issues.ย 

  • Checks any medications the patient may be taking to identify possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies and contraindicated medication.ย 

  • Conducts a complete medical history, family support systems, home environment and safety issues.ย 

  • Coordinate, implement and manage administrative functions and areas of programs related to nursing services which may include: documentation or medical records, nursing supplies, ancillary supplies or services, agency Pl; Care Plans; Infection Control; Skin Care Program; Special Programs; Staff Scheduling 'Selection, Training, Orientation or related functions.ย 

  • Communicates regularly with the ADON and other staff members to review problems or unique issues from weekend visits, share professional support, and exchange feedback aimed toward enhancing professional growth.ย 

  • Maintain a working knowledge and ensure compliance with all governmental regulations.ย 

  • Participate in Nursing Staff Meetings as requested.ย ย ย ย ย 

  • Adhere to all of the Company's policies and procedures.ย 

  • Perform other duties as assigned.ย
