About This Role

The Nurse Navigator will serve as a primary contact for oncology patients, caregivers, and referring physicians, facilitating access to clinical and supportive services throughout the oncology care continuum.

What You'll Do
  • Coordinate patient referrals to Hematology/Oncology.
  • Schedule appointments within established guidelines.
  • Collaborate with the multidisciplinary team for patient care.
  • Maintain professional relationships with physicians and staff.
  • Demonstrate a positive attitude aligned with clinic values.
  • Ensure workspace safety and adhere to safety regulations.
  • Comply with State, Federal, and local regulations and guidelines.
  • Stay current on job-related knowledge and skills.
  • Provide education and emotional support to patients and families.
  • Assist in coordinating end-of-life care for patients and families.
RequirementsEducation:
  • Bachelor's degree in nursing or equivalent supervisory/management experience (4 years)
Experience:
  • 2+ years of clinical RN experience
  • 2+ years of Oncology experience
Licenses & Certifications:
  • Current RN license or valid permit to practice professional nursing in Texas
  • BLS through American Heart Association required
  • ONC certification required within 12 months of hire
