Sunrise Services, Inc. is looking for aCare Coordinator & Navigation Specialist to join our Health Homes Program!

About the Program:

Health Homes is an outreach program which engages clients (including pediatrics) in the community with health and social services to best meet their needs. Health Home (HH) Services are intensive services that coordinate care across several domains. The purpose is to coordinate clinical and social service expertise for high risk clients with complex chronic conditions, mental health challenges, substance use disorder issues, and/or long term service needs and supports.

Duties & Responsibilities:

  • Work as a member of a multidisciplinary Health Homes Team

  • Assist Program Coordinator to processreferrals received fromLead Entities (North WestRegional Council, Molina, Amerigroup, CHPW etc.)

  • Assist Program Coordinator and Care Coordinators in screening and generating referrals where appropriate

  • Assist Program Coordinator in 'smart assignment' of clients to Care Coordinators

  • Assistin engaging hard to reach clients,establishingrapport andmaking arrangementsfor Care Coordinators to have in person contacts;

  • Carry out pre-engagement activities with clients and schedule face to face appointments for Care Coordinators in the field

  • Engage with clients via telephone and mail out activities to secure and confirm appointments with Care Coordinators

  • Assistcolleagues to obtain necessary consents andassistinidentifyinginitial clientobjectivesand goalsidentifiedat referral and duringinitialtelephone outreach.

  • Assist Program Coordinator in preparation for site visits and audits

Education/Experience:

  • Educationand experience:Must meet one of the followingcriteria:

    • Bachelorslevel degree in related mental health field with 2-years' experience
    • Behavioral Health Professional or Specialist- Mental Health Counsellor withMasterslevel degree in related field / Certified MH Counselor withBachelorslevel degree in related field / Agency Affiliated Counsellor withBachelors level degree in psychology
    • RegisteredNurse, Nurse Practitioner, Licensed Practical Nurse

    • Social Worker - Masters orBachelors in social work, human services or related field

    • Substance Use Disorder Professional

Schedule:

  • Monday - Friday 8 am - 4:30 pm

Licensure/Certifications:

  • Washington state driver's license
  • Current valid Washington driver's license with acceptable driving record and $50,000 automobile liability insurance required.

  • First Aid and CPR certification
  • Licensure related to qualifications listed above

Other Considerations:

  • DSHS background clearance including fingerprint
  • Acceptable driving record
  • $50,000 automobile liability insurance required
  • Strong verbal and written communication skills
  • Skilled with Microsoft Office Suite
  • Ability to lift approximately 20-30 pounds and physically assist unsteady clients

Compensation:

  • $22 - $25.50/hour
  • Wages are determined by applicable industry experience, education, and certifications/licensure.

    • Paid vacation - Earned at .04 per hour worked for hourly employees, and 6.67 per month for salary (equates to about 2 weeks per year for full-time employees)
