About the Program:
Health Homes is an outreach program which engages clients (including pediatrics) in the community with health and social services to best meet their needs. Health Home (HH) Services are intensive services that coordinate care across several domains. The purpose is to coordinate clinical and social service expertise for high risk clients with complex chronic conditions, mental health challenges, substance use disorder issues, and/or long term service needs and supports.
Duties & Responsibilities:
Assist Program Coordinator to processreferrals received fromLead Entities (North WestRegional Council, Molina, Amerigroup, CHPW etc.)
Education/Experience:
Educationand experience:Must meet one of the followingcriteria:
- Bachelorslevel degree in related mental health field with 2-years' experience
- Behavioral Health Professional or Specialist- Mental Health Counsellor withMasterslevel degree in related field / Certified MH Counselor withBachelorslevel degree in related field / Agency Affiliated Counsellor withBachelors level degree in psychology
Schedule:
Licensure/Certifications:
- Washington state driver's license
Current valid Washington driver's license with acceptable driving record and $50,000 automobile liability insurance required.
- First Aid and CPR certification
- Licensure related to qualifications listed above
Other Considerations:
- DSHS background clearance including fingerprint
- Acceptable driving record
- $50,000 automobile liability insurance required
- Strong verbal and written communication skills
- Skilled with Microsoft Office Suite
- Ability to lift approximately 20-30 pounds and physically assist unsteady clients
Compensation:
- $22 - $25.50/hour
- Wages are determined by applicable industry experience, education, and certifications/licensure.
