Job Title: Nurse Practitioner (Acute Care in the Home) - Choose Your Schedule

Location: Seattle Metro & Everett, WA (Field-Based)

Job Type: Full-Time, Part-Time, or PRN / Contract

  • Full-Time (3 x 12-Hour Shifts/Week): $165,000 - $175,000/year + Sign-On Bonus & Full Benefits (4 days off per week!)

  • Part-Time (2 x 12-Hour Shifts/Week): $99,000 - $105,000/year + Sign-On Bonus & PT Benefits (5 days off per week!)

  • Contractor / PRN (Minimum 1 x 12-Hour Shift/Week): $100/hour flat rate

The Role

This role brings your advanced clinical expertise beyond the walls of the hospital and into the home, where you can see the full picture. You'll practice with greater autonomy, deliver high-acuity care in a more personal setting, and use your clinical judgment to prevent complications, reduce hospitalizations, and make a lasting difference in your patients' lives.

You will practice at the top of your license-performing hands-on assessments, independent IV access, medication administration, wound care, and splinting completely autonomously. Reporting to the Field Provider Manager, you will have the freedom to assess, decide, and act.

What You Will Do
  • Deliver Care That Follows the Patient: Perform problem-focused assessments and deliver therapeutic interventions (e.g., IV fluids, medication administration) in the home. 

  • Own the Care Plan: Order and interpret diagnostic tests (labs, EKGs, POCUS), and collaborate with referring clinicians to create holistic care plans that integrate patient goals and palliative principles.

  • Improve Outcomes That Matter: Participate in quality initiatives that have a measurable impact, helping us achieve our 49% reduction in hospital readmissions and 97 NPS score.

Who You Are & Required Qualifications

Because of the highly autonomous, high-acuity nature of this role, we have strict clinical requirements. Please note: We are unable to accept new graduates or providers whose primary experience is in primary care, urgent care, telehealth, or aesthetics for this specific role.

  • Licensed Professional: Current, unrestricted state licensure as an NP (ARNP) or PA-C in Washington. You must have U.S. residency and be eligible to work without sponsorship.

  • Acute Care Veteran: You have a minimum of 2 years (or 3,600 hours) of clinical experience specifically in acute settings: Emergency Department, Inpatient Medicine (hospitalist, med-surg, telemetry, step-down), or Acute Post-Acute (LTACH, acute rehab, SNF).

  • Solo Procedural Fluency: You are comfortable operating as the solo on-site clinical authority. You possess demonstrated procedural fluency in independent IV access, medication administration, and point-of-care diagnostics.

  • Highly Desirable Certifications/Experience: AGACNP-BC, ENP-BC, or PA-C with NCCPA CAQ in Emergency or Hospital Medicine. 5+ years of acute practice, pre-licensure EMS background (paramedic/critical care transport), or previous Hospital-at-Home experience is a massive plus.

  • Mobile & Adaptable: You possess an active, unrestricted driver's license. You have the physical capacity to carry up to 60 lbs of equipment, navigate stairs, and deliver unpredictable, autonomous care in non-clinical home environments.

  • Certified & Flexible: Current BLS certification is required; ACLS is preferred. You are available for evenings, weekends, and holiday coverage to ensure our patients receive care when they need it most.
