Cardiology Advanced Practice Provider (Nurse Practitioner) Practice: Heart Clinic of Hammond Location: Hammond, LA Position Type: Full-Time | Clinic-Based with Hospital Rounding Schedule: Mondayโ€“Friday

Position Overview We are seeking a compassionate and motivated Nurse Practitioner to join Heart Clinic of Hammond in Hammond, Louisiana. The NP will work closely with Dr. Mikdadi to provide high-quality cardiovascular care, including outpatient clinic visits and hospital rounding at North Oaks Hospital and Post Acute Medical (PAM). This is an excellent opportunity to join a well-established cardiology practice with a strong emphasis on patient-centered, comprehensive care.

Key Responsibilities
  • Conduct outpatient clinic visits, including routine follow-ups, chronic disease management, and evaluation of ongoing cardiac concerns.
  • Support continuity of care by following patients post-hospitalization and integrating them into ongoing outpatient management.
  • Provide clinical support within the cardiology clinic, including patient education and assistance with diagnostic testing.
  • Collaborate with physician to ensure efficient patient flow and a well-coordinated clinic experience.
Hospital Rounding
  • Perform rounds at North Oaks Hospital and Post Acute Medical (PAM).
  • Support smooth transitions from hospital to outpatient care.
  • Participate in weekend call rotation, along with one major and one minor holiday annually.
Qualifications
  • Active Nurse Practitioner license in the state of Louisiana.
  • Prior cardiology experience preferred.
  • Strong clinical skills in both outpatient and hospital-based settings.
  • Ability to work independently while contributing to a collaborative, physician-led team.
