Advanced Practice Nurse

Job Title: Advanced Practice Nurse

Location: Barnabas Health Medical Group

Department Name: Roselle Park Medical Associates

Status: Salaried

Shift: Day

Pay Range: $125,000.00 - $195,000.00 per year

The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

Job Overview:

The Nurse Practitioner will work one day per week seeing patients in-office at Roselle Park Medical Associates, with the potential for afternoon home visits as needed for established home-patients within the area. The remaining four days will be spent providing care in a sub-acute nursing home setting. The position includes on-call responsibilities every fifth week, covering after-hours of 5:00 PM to 9:00 AM, Monday through Friday, and all day on weekends.

  • NP Certification
  • BLS and ACLS
  • CDS and DEA

Schedule:

  • Full Time
  • Days
  • 1 day in the office and 4 days at external facilities

Essential Functions:

  • Rounds regularly at one to two subacute care facilities
  • Performs transitional care visits for patients discharged from subacute/nursing home facilities
  • Conducts home visits as part of ongoing patient care
  • Preferred background or strong interest in palliative care
  • Completes POLST (Practitioner Orders for Life-Sustaining Treatment) forms
  • Collaborates closely with physicians within the practice to coordinate and align patient care
  • Performs comprehensive initial and episodic health assessments; in order to identify existing or potential health problems
  • Initiates and implements an individualized nursing and medical plan of care in collaboration with members of interdisciplinary team
  • Initiates appropriate diagnostic, therapeutic, medical and nursing actions in conjunction with physician
  • Provides follow-up care including acute and routine examinations
  • Assesses progress, identify problems, initiate preventive measures, recognize emergent problems, initiate proper care in collaboration with physician
  • Monitors and adjusts patients medication according to protocol or medical plan of care
  • Implements nutritional therapies, as appropriate
  • Triages patient problem calls which requires medical attention
  • Orders appropriate laboratory and diagnostic procedures
  • Recognizes normal and abnormal laboratory findings and the pathology involved
  • Documents information in patients charts following documentation standards
  • Provides health education to the patient and family (e.g., management of side effects, anticipatory teachings)
  • Manages medical emergencies as indicated, and according to standing orders
  • Provides monitoring and continuity of care between health care settings
  • Provides patient and family counseling and reassurance in regard to psychological and or psychosocial issues
  • Initiates proper referrals to private or community agencies
  • Acts as a resource and teacher for other members of the health care team
  • Shares knowledge and expertise with staff
  • Collaborates with the nursing and medical staff at partner and affiliate sites to coordinate continuity of care
  • Evaluates the quality of care as well as the quality of life for patients through assessment of patient response and recommend interventions, as indicated
