Job description

We are seeking a compassionate and dedicated Registered Nurse (RN) to join our healthcare team. The RN will be responsible for delivering high-quality nursing care to patients, coordinating with family members and physicians, and ensuring adherence to established policies, procedures, and safety standards.

Core Clinical Responsibilities

  1. Health Assessments:

    • Conduct initial and ongoing health assessments of participants.

    • Monitor vital signs and observe for changes in condition.

  2. Medication Management:

    • Administer medications as prescribed.

    • Educate clients and caregivers on medication use.

    • Maintain accurate medication records.

  3. Wound Care & Chronic Disease Management:

    • Provide basic wound care, catheter care, etc.

    • Monitor and manage conditions like diabetes, hypertension, dementia, etc.

  4. Emergency Response:

    • Respond to medical emergencies.

    • Coordinate with EMS or hospitals as needed.

Coordination & Care Planning

  1. Develop Care Plans:

    • Collaborate with other staff (social workers, physical therapists, etc.) to create individualized care plans.

  2. Communication:

    • Serve as liaison between participants, families, and healthcare providers.

    • Provide updates and education to family members as needed.

  3. Documentation:

    • Maintain up-to-date nursing notes and health records.

    • Document changes in condition, interventions, and outcomes.
