Job description

Service Coordinator Position - StepOne Service (offered by Evergreen Healthcare Partners, LLC)

StepOne Service is an inpatient, hospital-based 3 - 5 day Acute Withdrawal Management service available to adults requiring medical management of withdrawal symptoms from alcohol and/or opioids.

Acute Withdrawal Management is provided to individuals who are in impending or early stages of acute withdrawal at the time of hospital admission. The goal is to medically assist the individual through the withdrawal process and help navigate the transition to an appropriate level of community-based addiction treatment that can address the underlying causes of their substance use disorder.

StepOne Service has a fully qualified treatment team including an attending physician and nursing staff that utilizes a multidisciplinary team approach in the acute management withdrawal of the patients.

Position Purpose:

The Service Coordinator is responsible for planning activity and initiating contacts in the community (approximately 60 mile radius surrounding the client hospital) to enhance the visibility of the service and hospital. The Service Coordinator works collaboratively with hospital administrators, nursing leadership, and StepOne Service leadership team to create awareness in the community and coordinate a continuum of care for the patients with substance use disorders.

Essential Functions:

ยท Functions as a liaison between the contracted hospital and Evergreen Healthcare Partners, LLC

ยท Acts as a liaison between the service and the community

ยท Personal/Professional Development - Maintaining and expanding knowledge and understanding of the field of substance use disorders and social service

Typical Experience/Qualifications:

ยท Knowledge of substance use disorders and associated treatment

ยท Understanding of social services and/or hospital procedures and policies

ยท Familiarity with HIPAA guidelines and policies

ยท 2+ years of social service experience and/or clinical or inpatient services

ยท 2+ years networking and public relations that involved planned community education

ยท Experience in a self-motivated role

ยท License/Certified Credentials in the area of social services

ยท Ideal candidate will have associated experience and/or a bachelorโ€™s degree in the related field

ยท Live within a 60 mile radius of the client hospital
