• Conduct thorough psychosocial assessments to accurately identify patients' emotional and social needs, and formulate individualized care plans.
• Provide direct individual and group counseling sessions to patients coping with addiction, adjustment issues, grief, and other psychosocial challenges.
• Collaborate closely with medical professionals, including doctors and nurses, to integrate psychosocial considerations into comprehensive patient care.
• Coordinate post-discharge services with community organizations to facilitate seamless transitions from the detox center back to home or further treatment environments.
• Deliver education and support to patients and their families regarding treatment options and community resources, empowering them to participate actively in the recovery process.
• Participate in hospital committees to advocate for and enhance social work services within the organization.
• Develop effective discharge plans, ensuring that all patients are connected to the appropriate community resources to aid their recovery.
• Maintain precise and timely documentation of patient interactions in compliance with both legal and hospital standards.

Your primary focus will be on adults experiencing substance use disorders, undergoing detoxification, and facing accompanying mental health challenges. You will work with a diverse patient population, navigating the emotional and social hurdles associated with recovery, and offering vital support tailored to individual needs.

• A Master's degree in Social Work from an accredited program is essential.
• Must hold a current and active state licensure as a Licensed Clinical Social Worker (LCSW).

• A minimum of two years of experience in a healthcare or hospital setting specifically related to addiction treatment is preferred.
• Familiarity with crisis intervention strategies, mental health disorders, and interdisciplinary collaboration is highly advantageous.
• Demonstrated excellence in communication, interpersonal skills, and teamwork dynamics.
