CPSW Youth Services Navigator
FLSA: Full Time Salaried
Pay Rate: $41,600-$52,000
Serenity Mesa Recovery Center is an adolescent transitional living and recovery center in Albuquerque, NM, under the ownership and direction of Healing Addiction in our Community (HAC), a New Mexico non-profit corporation. Residents enter this program after completing drug rehabilitation and/or detoxification, for the purpose of ongoing recovery and transitional services.

CPSW Youth Services Navigator primary job responsibility is to develop and maintain a positive, healthy relationship with the residents. This position works directly with the residents to help them achieve their goals and objectives while in the program. This position also works with the clinical staff to ensure a well-rounded service plan and treatment program for each resident.
  • Key responsibilities include
  • working with residents individually to assist them with their overall service plan needs.
  • meeting with residents individually at least once per week assisting residents to enroll in school or GED program.
  • acting as primary contact with RFK Charter School for all things school-related.
  • working with residents to develop and maintain their Service Plan. Service Plans include any non-clinical goals residents have while at SM such as resume writing, budgeting, obtaining a job, enrolling in higher education, etc.
  • assisting residents in Medicaid/insurance enrollment and in obtaining food stamps, bus passes, and other needed community services.
  • assisting residents in obtaining their birth certificate, ID, SS Card, etc.
  • developing and maintaining a Life Skills Program for all residents.
  • ensuring that residents attend five (5) life skill classes per week utilizing a curriculum that matches resident needs.
  • providing transportation to residents as needed (work, school, doctor appts, etc.).
  • administering Life Skills Assessments to residents as required by funding sources. You must conduct a minimum of one life skill a week and two every other week.
  • Entering meds and profiles to Quick Mar upon intake and create a face-sheet to be uploaded to Teams.

Non-essential Job Functions include:
  • working directly with Program Manager and Director of Programming amp; Facilities on resident scheduling and other resident needs in the lodges.
  • providing coverage for the Intake Coordinator/Administrative Assistant as needed.
  • answering SM main phone lines and responding to emails as needed.
  • providing transportation to residents as needed.
  • working with Medicaid and other insurance providers for documentation needs.
  • providing coverage for other positions as needed.
  • providing on-call coverage as part of the on-call rotation.

Essential/Preferred Requirements (Education, Licensing, Experience)
  • Certified Peer Support Worker (CPSW Certification)
  • At least one (1) year prior case management experience is mandatory.
  • Bilingual (Spanish/ English) preferred
  • Above average proficiency in Word, Excel, and Outlook is essential
  • Ability to pass drug screening and all background checks is essential
  • Ability to pass motor vehicle record check is essential
  • Fluency in verbal and written English communication is mandatory
  • Proficiency in utilizing standard office equipment is essential
  • Time management and coordination of services mandatory
