About Us

We will provide a personal level of healthcare and addiction treatment that addresses the mental, physical, and spiritual well-being of everyone we touch. SSTAR is a Federally Qualified Health Center (FQHC) which affords our employees eligibility to apply for one of our three Loan Repayment Programs, including the HRSA LRP.
The Outreach Worker In Project Aware conducts outreach among vulnerable populations, provides targeted secondary services and assists Provider to meet clients' needs both on the medical unit and in the community. This role also includes responsibility for safely operating and transporting the outreach van or RV to and from designated outreach locations.
Must have reliable transportation and a valid driver's license.
Shift: Monday - Thursday 12:00pm - 4:00pm and Friday 8am - 4pm

Duties & Responsibilities
  • Developing and sustaining positive relationships with a range of people, including residents, community leaders, local groups and organizations, and local authorities
  • Interact with clients to determine their level of knowledge around harm reduction and their attitudes/ behaviors about HIV/HCV/STIs
  • Provide overdose education and distribute naloxone to both clients and the community
  • Assess potential barriers to care and work with clients to reduce barriers resulting from lack of transportation, mental illness, substance use, etc.
  • Link clients to medical care and appropriate treatment.
  • Initiate appropriate client referrals both internally and externally to the organization.
  • Collect and accurately enter program activity data into the appropriate system in a timely manner
  • Attend relevant training courses and participate in all assigned regional meetings.
  • Stay informed about current trends, development, and research
  • Adheres to the work schedule. Obtains supervisor approval in advance of schedule conflicts as well as late arrivals or early departures.
  • Records all hours worked by the last day of the pay period.
  • Responds appropriately to all Code Calls
  • Follows the incident reporting policy and procedure in reporting any unsafe/unusual safety issues.
  • Treats patients, and colleagues with dignity and respect; honors each person's right to privacy/ maintains very strict patient and colleague confidentiality.
  • Timely completes all trainings, certifications, re-credentialing and licensing renewals requirements.
  • Maintains current awareness of SSTAR's communications by checking, reviewing and
    responding to work emails at minimum on a weekly basis. Asks for assistance when needing help
    or to fully understand the email message.
  • Performs other duties/special projects assigned.

Pay range: $18.00 - $21.00/hr
Experience and Skills
Education & Experience
  • HS Diploma/GED
  • Two years of full-time experience in the field of health care, medical care or public health education in a recognized hospital, medical facility, or agency.

Pay Rate Range: $18.00 - 21.00/ hour
