Job description

Supports the Activity Director or Assistant Activity Director with facilitation of activity programs designed to meet the interests and physical, mental, and psychosocial well-being of each patient. Helps create an engaging environment that fosters a strong sense of community and belonging.

Essential functions and responsibilities:

The essential functions of this position include the following. This list is not intended to be exclusive or exhaustive. There may be additional essential functions based on the unique nature of the position or environment.

  • Facilitates activity programs as planned to meet the functional levels, needs and interests of each Resident.
  • Assist Resident to and from activity programs and outings as scheduled, as required.
  • Maintains good rapport with family members and encourages family support in programs.
  • Completes appropriate records indicating Resident’s attendance and participation in individual and group activities in a timely basis.
  • Provide input to the assessment and evaluation of Residents' participation when requested. Communicates Resident’s concerns and responses to interventions to immediate supervisor.
  • Supports Residents' individual needs, preferences, customary routines, concerns and choices.
  • Assists with escorting Residents to and from activities.
  • Assist and work with individual volunteers and volunteer programs at the request of the Activity Director.
  • Cleans equipment and materials used for activities.
  • Participates in long-term care survey process, regulatory, and accreditation requirements.
  • Performs other duties as assigned.

Qualifications:

  • Education and/or Experience:
  • Minimum high school diploma or general education degree (GED)
  • Previous experience in activities, recreation therapy, and long-term care preferred.
  • Computer skills are required.
  • Meets state and federal criminal background check requirements.
