As a Licensed Practical Nurse (LPN) at Rivers Edge Nursing & Rehab, you will play a vital role in providing high-quality care to our residents. As a valued member of our team, you will work closely with our dedicated staff to deliver exceptional patient-centered care.

About the Role:

  • Licensed Practical Nurse (LPN)

  • As a Licensed Practical Nurse (LPN), you will be responsible for providing direct patient care, administering medications, and performing various nursing procedures under the supervision of our experienced nursing staff.

  • You will work closely with our interdisciplinary team to develop and implement personalized care plans, ensuring our residents receive the best possible outcomes.

Responsibilities:

  • Provide direct patient care to residents in long-term care facilities, including administering medications, performing wound care, and bathing.

  • Develop and implement individualized care plans in collaboration with the healthcare team.

  • Monitor and record resident vital signs, medical conditions, and any changes in their condition.

  • Collaborate with other healthcare professionals to ensure comprehensive care and address resident concerns.

  • Maintain accurate and up-to-date records of patient care and medical treatments.

Requirements:

  • LPN licensure in the state of Wisconsin
