Compensation:$19-$21/hr

Status:Full-Time, W2 Employee

Schedule:Monday - Friday, 8 AM - 5 PM

About the Role

At Aegis Healthcare, our Bridge Program supports patients living with serious illness before they need hospice. As a Bridge Care Coordinator, you'll be a steady, compassionate presence in patients' lives, helping them and their families navigate their healthcare journey with dignity, education, and understanding.

This is a relationship-driven role for someone who cares deeply about older adults, works well independently, and builds trust naturally with patients, caregivers, and healthcare partners alike.

Your key responsibilities will include:

  • Build lasting relationships through non-clinical visits in homes, assisted living communities, and skilled nursing facilities.
  • Offer emotional support and companionship while honoring each patient's goals and independence.
  • Educate patients and families on disease progression, community resources, and advanced care planning.
  • Serve as a trusted point of contact, recognizing changes in patient needs and sharing them with the clinical team.
  • Collaborate with physicians, nurses, social workers, and facility partners to keep care coordinated and patient-centered.
  • Maintain accurate, timely documentation of every interaction.
