Job description

Our company is an established home healthcare company that has been servicing patients in Illinois seeking a high-energy, self-initiating, and caring RN/LPN who can work as a field nurse for the city of Chicago North side 60619, 60620, 60628, 60643 and Chicago south east suburbย etc..... Your organizational skills and strong understanding of home health, geriatric care, and previous experience in home health will be highly valued and very well compensated.

This is a full time position but Part time or PRN are welcome to apply too.ย 

QUALIFICATIONS:ย 
1. ย  ย Graduate of a state approved school of practical (vocational) nursing and currently licensed in the state(s) in which practicing.ย 
2. ย  ย Minimum of one (1) year experience in nursing, preferred.ย 
3. ย  ย Acceptance of philosophy and goals of this Agency.ย 
4. ย  ย Ability to exercise initiative and independent judgment.ย 
RESPONSIBILITIES:ย 
1. ย  ย Understands and adheres to established policies and procedures.ย 
2. ย  ย Implements the nursing care plan for each patient.ย 
3. ย  ย Provides nursing services, treatments and diagnostic and preventive procedures as assigned.ย 
4. ย  ย Initiates preventive and rehabilitative nursing procedures as appropriate.ย 
5. ย  ย Observes signs and symptoms and reports to the physician and RN reactions to treatments, including drugs and changes in the patientโ€™s physical or emotional condition.ย 
6. ย  ย Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.ย 
7. ย  ย Evaluates with registered nurse the effectiveness of the LPNโ€™s/LVNโ€™s nursing service to the patient and family under the guidance of the registered nurse.ย 
8. ย  ย Maintains accurate and complete records of observations, treatments and care of patient.ย 
9. ย  ย Participates in medical record audit as assigned.ย 
10. ย  ย Attends staff meetings, patient care conferences and in-services as scheduled.ย
