Allure of Walnut Skilled Nursing Facility – Full-Time and Part-Time

Location: 308 S Second Street, Walnut, IL 61376

Shifts Available

Full-Time First Shift: 7:00 AM – 3:00 PM

Part-Time Third Shift: 11:00 PM – 7:00 AM

Position Summary

The RN/LPN provides skilled nursing care to residents under the direction of the attending physician and in accordance with state nursing practice regulations. This role ensures compliance with facility policies while delivering compassionate, resident-centered care.

Essential Duties & Responsibilities
  • Provide nursing services in accordance with scope of practice, facility policies, and professional standards

  • Monitor residents for acute changes in condition and notify providers as appropriate

  • Monitor and manage chronic health conditions and report significant changes

  • Assist with admissions, resident orientation, and discharge planning

  • Ensure physician orders are in place for newly admitted residents

  • Participate in MDS assessments (admission, quarterly, change of condition, etc.)

  • Contribute to individualized, resident-centered care plans with the IDT

  • Provide culturally competent, trauma-informed nursing care

  • Monitor resident weight, nutrition, and hydration status

  • Supervise and provide guidance to CNAs as directed by the Director of Nursing

  • Maintain accurate nursing documentation in EMR systems

  • Collaborate with hospice and palliative care teams

  • Promote fall prevention and resident safety initiatives

  • Administer medications and monitor for adverse effects or errors

  • Transcribe provider orders accurately

  • Assess and manage resident pain

  • Assist with ADLs and resident repositioning as needed

  • Monitor skin integrity and provide wound care as ordered

  • Collect specimens for laboratory testing

  • Adhere to infection prevention and control protocols

Qualifications
  • Graduate of an approved RN or LPN program

  • Current nursing license in good standing (Illinois)

  • CPR certification required

  • Long-term care experience preferred

Physical & Sensory Requirements
  • Ability to move throughout the workday

  • Ability to read, write, speak, and understand English

  • Ability to manage mental and emotional stress

  • Ability to function independently with flexibility and professionalism

  • Ability to lift, push, pull, or move at least 25 pounds

  • Ability to assist with resident evacuation in emergencies
