Healthcare Consultant I

Job Duration: + Months (Possibilities of extension)

Job Location: Miami FL 33135 Candidates must reside in FL within one of the following ZIP codes: 33126, 33125, 33130, 33135, 33142

Training: Remote via Microsoft Teams (approximately 4–6 weeks)

Travel: 50–75% travel within the Mid-Miami region for in-home visits, assisted living facilities, and nursing homes.

Position Summary

We are seeking a self-motivated, energetic, detail-oriented, highly organized, and tech-savvy Bilingual Case Management Coordinator to join our Case Management team. This role is responsible for assessing, planning, implementing, and coordinating case management services through both telephonic and face-to-face interactions. The Case Management Coordinator manages a caseload of supportive and medically complex members, ensuring appropriate healthcare services, care coordination, and improved health outcomes.

Required Qualifications

Fluent Bilingual Spanish/English (reading, writing, and speaking) Bachelor's degree required Social Work or related field preferred No nursing degrees Minimum 1 year of Case Management experience Ability to multitask, prioritize, and adapt in a fast-paced environment Strong verbal and written communication skills Proficient with Microsoft Office (Word, Excel, Outlook) Must reside within the required Mid-Miami ZIP codes

Experience Requirements
