Title: Nurse Practitioner/Physician Assistant - Primary Care Clinic

About Us: WithYou MD is a primary care clinic that has both strong mental/behavioral health. We are a technology-forward practice, utilizing telemedicine and remote patient monitoring when appropriate. A key treatment of ours is Medication-Assisted Treatment (MAT) for Substance Use Disorders (SUD). Unlike a lot other practices that offer MAT, we incorporate a comprehensive treatment plan that includes intensive therapy for our patients. We are currently seeking a highly motivated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) or to join our dynamic team and provide virtual care to our patients. Experience prescribing Medication-Assisted Treatment (MAT) is highly preferred. Key Responsibilities:

• Conduct virtual patient visits on telemedicine online platforms.
• Work with patients and pharmacies to ensure patients receive the prescriptions needed.
• Provide comprehensive patient care under the supervision of a physician, including diagnosis, treatment, and management of acute and chronic illnesses.
• Develop and implement patient management plans, record progress notes, and assist in continuity of care.
• Educate and counsel patients on health maintenance and disease prevention.
• Collaborate with other healthcare professionals to ensure high-quality patient care.
• Participate in quality improvement initiatives and keep abreast of advancements in the medical field.

Qualifications:

• Current Physician Assistant (PA) or Nurse Practitioner (NP) license and certification in the state of New York.
• At least one year of experience in primary care. Bonus points if experience with suboxone/MAT prescriptions and providing high-quality care to MAT patients.
• Strong clinical and diagnostic skills.
• Excellent communication and interpersonal skills.
• Ability to work both independently and as part of a team.
• Commitment to ethical practice and patient confidentiality.
