MDS Coordinator

Job Summary:

Wedgemere Health Care, a premier healthcare provider, is seeking a dedicated and compassionate MDS (Minimum Data Set) Coordinator to join our team in Taunton, MA. As a key member of our healthcare team, you will play a vital role in ensuring the accurate and timely completion of MDS assessments, thereby contributing to the highest level of resident care. Responsibilities:

• Conduct MDS assessments for residents in a timely and accurate manner, in compliance with federal regulations and company policies
• Collaborate with the healthcare team to ensure accurate and complete data collection, including reviewing medical records and obtaining necessary information from healthcare providers
• Develop and maintain complex care plans, incorporating resident goals and preferences
• Coordinate with care teams to ensure seamless transitions and effective ongoing care
• Participate in Quality Improvement initiatives to enhance resident outcomes and reduce risk
• Develop and maintain strong relationships with residents, families, and staff to ensure exceptional customer service

Requirements:

  • 2 years of experience in geriatric care or a related field (training and mentorship provided)
