Job SummaryDelivers high-quality primary and preventive care for adults in an integrated community-mental-health setting, focusing on chronic-disease management, wellness promotion, and collaboration with behavioral-health teams.
Key Responsibilities- Conduct full health assessments, diagnose & manage acute/chronic conditions.
- Provide medication management, patient & family education, and minor procedures.
- Order/interpret diagnostic testing.
- Collaborate with physicians, therapists, case managers; mentor nursing staff.
- Maintain thorough EHR documentation and participate in qualityimprovement initiatives.
Qualifications- MSN or DNP, national NP board certification (AANP/ANCC).
- Active IL NP license; Controlled Substance/DEA & FullPractice Authority preferred.
- ≥12 yrs primarycare NP experience; behavioralhealth exposure valued.
- Proficiency with EHR (athenaOne) and strong teamwork/communication skills.
- Work Environment & Physical Requirements
- Ambulatory clinic environment with some standing/walking; must meet standard clinical credentialing and vaccination requirements.
