Key Responsibilities
  • Conduct comprehensive psychosocial assessments to identify patients’ emotional, social, and financial needs, collaborating effectively with medical teams to formulate holistic care plans.
  • Provide individual and group counseling sessions for patients facing a variety of challenges, including emotional adjustment, grief response, and coping with chronic illnesses.
  • Engage in crisis intervention strategies to support patients and their families during critical moments, ensuring timely access to necessary resources.
  • Develop discharge plans tailored to each patient's needs, coordinating with community organizations to facilitate post-discharge services and ensure continuity of care.
  • Document all patient interactions accurately and promptly in compliance with legal requirements and hospital policies, maintaining confidentiality and ethical standards.
  • Participate in interdisciplinary rounds and committees, providing insights on social work-related matters to enhance overall patient care and satisfaction.
  • Educate patients and their families about available treatment options, community resources, and strategies for self-advocacy within the healthcare system.
Patient Population / Client Focus

In this role, you will work with a diverse patient population experiencing a range of challenges, including substance use disorders, mental health issues, and general healthcare needs. The patients you support will often require a compassionate approach, understanding their unique circumstances and the intersection of their physical and emotional health.

Qualifications
  • A Master’s degree in Social Work from an accredited program.
  • An active state license as a Licensed Clinical Social Worker (LCSW).
Preferred Experience
  • Two years of experience working in a healthcare or hospital setting, particularly in acute care or substance use recovery.
  • Familiarity with medical terminology, mental health disorders, and crisis intervention techniques.
