Position Overview

The Wellness & Insight Therapies Center is seeking a dedicated and experienced Licensed Clinical Social Worker (LCSW) to join our team in a full-time remote capacity. This pivotal role requires a passionate individual who can provide critical support and resources to clients facing various behavioral health challenges. Work Environment

This position is predominantly remote, providing the flexibility necessary for work-life balance while still engaging effectively with clients and team members. The clinical team operates in a supportive and collaborative culture, ensuring that every professional has access to resources and mentorship that enhance their practice. While remote, there will be occasions for required travel to our Hospice Center and other associated sites for team meetings, retreats, and professional development opportunities.

Key Responsibilities

• Conduct thorough assessments to determine patient needs and collaborate with the clinical team to develop appropriate treatment plans.
• Facilitate referrals to additional services, including community-based self-help programs, to ensure clients receive comprehensive care.
• Engage in regular meetings with leadership to discuss case reviews, clinical challenges, and operational issues, ensuring continuity in patient care and effective team collaboration.
• Supervise and guide the daily activities of behavioral health therapists, conducting annual performance reviews and upholding departmental policies and procedures.
• Maintain accurate and detailed clinical productivity records in line with organizational expectations while actively participating in quality assurance and performance improvement initiatives.

Patient Population / Client Focus

The ideal candidate will work with a varied patient demographic, including individuals facing mental health crises, chronic illness, and end-of-life considerations in our hospice setting. The ability to empathize and build rapport with clients from diverse backgrounds is paramount, as is the commitment to adherence to best practices in social work.

Qualifications

• A Master's degree in social work is required, along with an active Licensed Clinical Social Worker (LCSW) certification.
• A strong ability to work in a medical setting, demonstrating adeptness at collaborating with healthcare practitioners and other service providers.
• Willingness to travel occasionally for team meetings and professional events is essential.

Preferred Experience

• A minimum of three years of clinical experience diagnosing and treating behavioral and psychiatric conditions is required.
• Experience in palliative care or hospice environments can be a significant asset.
• Proven track record of engaging in multidisciplinary teamwork and effectively communicating with clients and healthcare professionals.
