St. Joseph's Center is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our nursing team in a community-based Intermediate Care Facility home in Swoyersville, PA. This home supports residents in a community setting, providing a safe and nurturing environment where dignity and independence are prioritized.
This position is part-time 1st shift (7am-3pm)

What You'll Do
As an LPN at St. Joseph's Center, you'll play a vital role in ensuring the health and well-being of our residents.
  • Provide direct nursing care and health monitoring for residents.
  • Administer medications accurately and monitor for side effects.
  • Develop, implement, and evaluate individualized care plans.
  • Maintain medical records and ensure compliance with ICF/ID regulations.
  • Assist residents with activities of daily living (hygiene, dressing, feeding).
  • Accompany residents to medical and dental appointments.
  • Respond to health emergencies and implement appropriate interventions.
  • Collaborate with the interdisciplinary team and participate in care planning.
  • Maintain adequate medical supplies and equipment.
  • Support residents in community activities and outings.

What's Required of You
  • Current LPN license in Pennsylvania
  • CPR certification
  • Valid PA driver's license
  • Experience with individuals with intellectual and developmental disabilities preferred
  • Ability to lift/move 25-50 lbs and assist with transfers
  • Strong communication and teamwork skills
  • Commitment to person-centered care and St. Joseph's mission
