Sawyer County Health and Human Services is seeking a dedicated, empathetic, and organized professional to serve as a full-time ADRC Specialist. Under the direct supervision of the Adult Long-Term Supervisor, you will act as a vital community gateway for the Aging and Disability Resource Center of the North (ADRC-N). In this role, you will connect adult Sawyer County residents, including the elderly and individuals with physical or intellectual/developmental disabilities, with public and private long-term care programs. Our department is deeply committed to maximizing self-sufficiency and choice, integrating trauma-informed care into every community interaction.
Qualified candidates will have a Bachelor’s degree from an accredited human services program, a related field, OR a valid license to practice as a Registered Nurse (RN) in the State of Wisconsin. A minimum of 1-year experience working with adults in ADRC target groups (elderly or adults with physical or intellectual/developmental disabilities). Qualifying work experience may be paid or unpaid and may include internships, field placement, and volunteer work. Knowledge of and/or experience with public benefits (e.g., Medicare, Medicaid, SSDI, etc.) preferred.
You must possess excellent assessment skills and organizational skills. Equally important is your ability to work independently in challenging situations. You will need an aptitude for communication, both verbal and written.
The successful candidate will be required to become certified in the Wisconsin’s Long Term Care Functional Screen (LTCFS) within three months of employment. New hires are required to hold a valid drivers’ license, pass a pre-employment drug screen and background check.

Sawyer County Position Description
Title: ADRC Specialist
Department: Sawyer County Health and Human Services, Aging and Disability Resource Center, Sawyer County Branch
Reports to: Adult Long Term Care Unit Supervisor
Hours: Fulltime (40 hrs/wk)

Position Purpose:
Under the direct supervision of the Adult Long Term Supervisor, the ADRC Specialist through the Aging and Disability Resource Center of the North (ADRC-N)
provides information about services, programs, resources (public and private) to meet the long term care needs of adult Sawyer County residents. As such, the ADRC Specialist can help maximize opportunities for self-sufficiency and choice for target groups, including adults with physical and/or developmental disabilities and elderly, The ADRC Specialist is expected to integrate trauma informed care while serving our communities.

Essential Duties and Responsibilities:

Provide ADRC customers with information about services, programs, resources (public and private) that can help maximize opportunities for self-sufficiency and choice.
·
Provide ADRC customers with assistance in connecting with the appropriate resources and services to meet their needs.
·
Provide follow up with customers to determine if needs were met or if additional assistance is necessary.
·
Provide long-term care options counseling.
·
Provide referrals to appropriate agencies and/or resources as necessary.
·
Provide enrollment counseling to ADRC customers who are eligible for publicly funded long-term care services.
·
Consult with long-term care functional screen lead liaison as necessary to perform functional screening for potential publicly funded long term care services
·
Determine the range of needs through motivational interviews, home visits, collateral contacts, etc.
·
Prepare SSI-E certifications.
·
Complete training for and administer Memory Screens
·
Knowledge of resources available to target populations served.
·
Support Aging and Disability Services Section and regional ADRC of the North.
·
Record and maintain complete and accurate records and perform other general office duties as required.
·
Provide informational presentations to other staff, professionals, and community groups related to department programs as needed.
·
Participate in section and department meetings, work-related committees, and trainings.
·
Participate in supervision/consultation.
·
Provide information to mental health and AODA target groups.
·
Answer ADRC Regional phones as scheduled.
·
Participate in and/or facilitate ADRC initiatives such as Dementia Friendly Community, Music and Memory, Powerful Tools for Caregivers, Purple Tube, etc.

Education and Experience:
Minimum of a bachelor’s degree from an accredited human services program, a license to practice as a registered nurse in Wisconsin pursuant to s. 441.06 Stats, or related field. A minimum of 1-year experience working with adults in ADRC target groups
(elderly or adults with physical or intellectual/developmental disabilities). Qualifying work experience may be paid or unpaid and may include internships, field placement, and volunteer work.
Knowledge of and/or experience with public benefits (e.g., Medicare, Medicaid, SSDI, etc.) preferred. Experience in computer use and keyboarding. Valid Wisconsin Driver's License and proof of insurance. Applicants must meet Wisconsin’s Long Term Care Functional Screen (LTCFS) screener qualifications and successfully complete certification within 3 months of hire. Maintain LTCFS screener status via bi-annual recertification of Wisconsin’s LTCFS. Applicants must successfully complete ADRC Specialist training. Applicant must be able to manage multiple programs, and provide outreach and presentations to the community and area agencies.
