Job Summary

At Advanced Rehabilitation Center of Tustin, we are dedicated to delivering exceptional medical and nursing care in a warm and home-like environment. As a Social Services Director, you will play a vital role in ensuring our residents receive comprehensive support and services that cater to their medical, psychosocial, and spiritual needs. We are seeking a compassionate and experienced Social Services Director to lead our social services team in providing individualized care and support to our residents. As a key member of our management team, you will be responsible for developing and implementing social services programs, coordinating with healthcare professionals, and ensuring compliance with regulatory requirements.

Responsibilities

• Develop and implement social services programs that promote resident well-being, independence, and quality of life
• Collaborate with healthcare professionals to identify resident needs and develop comprehensive care plans
• Conduct social assessments, develop plans of care, and provide supportive services to residents
• Coordinate with family members and caregivers to ensure effective communication and support
• Supervise and mentor social services staff to provide exceptional resident care
• Maintain accurate records and reports, and ensure compliance with regulatory requirements
• Participate in quality improvement initiatives to enhance resident care and services

Requirements

• 1+ years of experience in social services or a related field
• Degree in Social Work, Psychology, or a related field (or equivalent experience)
• Proven leadership and management skills, with experience in supervising social services staff
• Excellent communication, interpersonal, and problem-solving skills
• Ability to work in a fast-paced environment and prioritize tasks effectively
